Answering 'why are you leaving?'
Stay factual and forward-looking: what you have outgrown, and what you want next. Never criticise a former manager or employer, even fairly.
Answering the weakness question
Name a real, non-fatal weakness, then describe the system you use to manage it. 'I under-communicate progress when heads-down, so I now send a Friday status note to my stakeholders.'
